One of our office staff will reply with a CC AUTHORIZATION FORM including information
needed to pay the Banamex DD visa fee (VISA or MC are accepted)
Applicant will fax or scanned email the cc authorization form to FAX: 619-374-2298 Email:

Once the cc payment is processed one of our office staff will reconfirm via email including
delivering address.

Note: Our office needs to receive item #3 via fax or scanned email five working days prior to your
appointment date. You can also pay with cashier check same rule applies we need to receive it five
